South Carolina updates Medicaid paperwork for incontinence supplies

United States

The South Carolina Department of Health and Human Services announced on October 2 that it had updated the Prescriber Certification of Incontinence Form (168IS) for service dates on or after October 1, 2026. The form now asks for the prescribing provider's individual National Provider Identifier (NPI), alongside other required information including the diagnosis related to incontinence and confirmation of medical necessity.

Suppliers must keep the completed, signed form in patient records and include the prescriber's NPI on claims. The agency says incontinence-supply services cannot be reimbursed from December 1, 2026, if that NPI is missing from the claim. Prescribers must also meet South Carolina Medicaid enrolment requirements.

This is a billing and documentation change for providers serving eligible members in South Carolina. The notice does not announce a nationwide change to US product coverage.
